Background: Cataract remains the leading cause of reversible blindness worldwide, significantly impairing visual function and quality of life, particularly among the elderly population. Advances in surgical techniques have made cataract extraction highly effective and safe. Phacoemulsification (Phaco) and small incision cataract surgery (SICS) are the two most commonly performed procedures.
Aim: To compare postoperative visual outcomes in patients undergoing phacoemulsification and small incision cataract surgery.
Methods: A prospective, comparative study was conducted on 126 eyes of 126 patients, randomly allocated into two equal groups: 63 underwent phacoemulsification and 63 underwent SICS. All surgeries were performed by a single experienced surgeon to maintain consistency. Visual acuity was recorded preoperatively and postoperatively on Day 1, Day 7, Day 28, and Day 84 to assess the speed and stability of visual recovery.
Results: Visual acuity on postoperative Day 1 and Day 7 was significantly better in the phacoemulsification group (p < 0.001), indicating faster visual rehabilitation. However, by Day 28 and Day 84, no statistically significant difference was observed between the two groups (p > 0.05), demonstrating comparable long-term visual outcomes.
Conclusion: Phacoemulsification provides superior early postoperative visual recovery; however, both phacoemulsification and SICS achieve similar visual outcomes in the long term. Considering its cost-effectiveness, safety, and efficiency, SICS remains an excellent alternative, especially in resource-limited and high-volume clinical settings.
